The primary reason for needing to unsign a SOAP note you've previously signed would be to make necessary edits. You can only edit a signed note by first unsigning it.
Only the provider associated with a SOAP note has the ability to sign or unsign that note.
Here's how to unsign a SOAP note:
1) Navigate to the SOAP note which you would like to edit.
2) From the SOAP note, you will click the unsign note button. (hint: it's the same button you used to sign the note!).

3) Make any necessary edits, but be sure to sign & save the note again!
Note!
If you have previously printed the note for any reason other than in-office use, OR if the note has a corresponding claim submitted, then you will be alerted prior to unsigning.
