Send groups of surveys

What are intake groups? 

In ChiroUp, you'll find two intake groups: Basic Intake and Detailed Intake. These groups are designed to streamline your intake process by enabling you to send multiple surveys with just one selection. 

Simply choose an intake group when sending a survey, and the corresponding follow-up surveys will automatically populate for your patients. See below for the surveys included in each group:

  • Basic Intake Group: includes the Chief Complaint Survey, Health History, and Review of Systems. The basic group is designed for simple presentations or re-activations of existing patients. 
  • Detailed Intake Group: includes the Chief Complaint Survey, Patient Demographics, Family History, Health History, Lifestyle History, Medication History, Review of Systems, and Surgical History. The detailed group is designed for new patients. 

 

Can I modify the surveys in each group? 

Absolutely! We've set default surveys to populate, but if you'd like to make the intake groups your own, simply navigate to your Clinic Settings and click on Patient Preferences. Towards the bottom of the page, you'll find options to add or delete surveys in the various intake groups.
