Collecting basic demographic information for your patients has never been easier in ChiroUp!

You can send a demographic survey at any time from the patient record, or when adding a new patient!  

From the patient record:

1) Click on "Request Info"

2) Select "Patient demographics" from the menu of survey options.

3) Confirm your method of delivery and click "Send now".

When adding a new patient:

1) Click on "Request Info"

2) Select "Patient demographics" from the menu of survey options.

3) Confirm the destination and click "Create".
