Tracks allow you to maintain separate documentation for different types of care within the same patient record. They are commonly used by clinics that provide multiple types of care, such as Chiropractic and Physical Therapy.
Tracks are OFF by default for new clinics, so this article will help you determine whether Tracks are right for your workflow and, if so, how to turn them ON for your clinic or an individual patient. You’ll also find links to additional Tracks training and resources.

What is a patient Track?
A Track acts like a dedicated folder for a patient's SOAP notes and care plan related to a specific complaint or set of related complaints.
For example, if a patient has back and rib pain from a car accident, their provider can have a Track containing the notes and care plan related to that treatment. If the same patient also receives a different type of care, such as Massage Therapy, a separate Track can keep that care documented independently.
Why use Tracks?
Tracks help keep documentation for different types of care separate within the same patient record. They keep related complaints, SOAP notes, and care plans connected across visits, making it easier to document and manage ongoing care.
Tracks do not prevent providers from sharing notes. Providers can access and document within the same Track when appropriate, allowing multiple providers to collaborate on the same course of care.
How do I know if my clinic needs to use Tracks?
Tracks are most useful when your clinic provides multiple types of care and patients may receive those types of care at the same time. Separate Tracks keep each type of care organized within the patient's record.
🌟 Multiple types of care? We recommend enabling Tracks clinic-wide. For example, a clinic offering both Chiropractic and Physical Therapy may use separate Tracks for each type of care.
🌟 Only one type of care? We recommend leaving Tracks OFF. This is true even if patients see multiple providers. Multiple providers can work within the same Track when appropriate.
Tracks can also be enabled for an individual patient when separate documentation is needed for a specific type of care, such as a Personal Injury or Workers' Compensation case. However, using a separate Track for these cases is optional based on your personal preference.
How do I turn Tracks ON for my whole clinic?
Navigate to Clinic Settings > General.
Toggle ON Use Tracks.

How do I turn Tracks ON for an individual patient
Navigate to the General tab of the patient's record and click Edit.
Scroll down and toggle ON Use Tracks.

What happens when I turn Tracks on for the first time?
When you turn Tracks ON for the first time, ChiroUp will handle existing patients based on whether they have active complaints:
- Patients with active complaints: ChiroUp will create one Track that includes all of the patient’s active complaints. If you need to create additional Tracks, you can do so manually from the Manage Tracks screen on the patient record or by selecting Create new Track when booking an appointment.
- Patients without active complaints: Nothing happens right away! The next time the patient checks in for an appointment, ChiroUp will create a brand-new Track based on the complaint they report on their Chief Complaint Survey at that time.
