Tracks allow you to keep notes and complaints separate when a patient receives different types of care in your clinic. For example, a patient may see a chiropractor for low back pain and a massage therapist for shoulder pain. With Tracks, each provider can maintain a separate “folders” (tracks) for their respective care.
If your clinic or a patient no longer needs separate Tracks, you can turn Tracks OFF. This combines the patient’s care into one unified set of notes moving forward. You can elect to turn off tracks clinic-wide or for individual patients.
⚠️ This article explains how to turn Tracks OFF at the clinic or patient level, what happens to existing Tracks when you make the change, and what to consider before turning Tracks OFF.

🧑💻 What happens when I turn Tracks OFF?
When you turn Tracks OFF, new encounters are documented in one unified set of notes rather than separate Tracks. Each new note carries forward information from the patient’s previous visit, creating one continuous record of care moving forward.
Because a patient may have multiple active complaints across their existing Tracks, ChiroUp will prompt you to review those complaints and decide which should continue as part of the patient’s care. Any complaints you keep, along with their Assessment information, will carry forward from visit to visit, regardless of the provider or type of care. Your Objective findings will carry forward based on the Objective input of the prior encounter.
What happens to existing Tracks? Existing Tracks are inactivated, not deleted. This allows you to turn Tracks back ON later if needed. All previous notes are preserved and can still be edited, signed, or unsigned as usual.
‼️ Turning Tracks OFF for your Entire Clinic
Here's what happens when you turn tracks off for your entire clinic in Clinic Settings > General > Use Tracks.
At the patient's next encounter:
- Their subjective survey (interim or re-exam) will only ask about complaints from the patient's most recent encounter.
- If the patient has any other active complaints (usually from other tracks) that weren't included on the most recent encounter, ChiroUp will prompt the provider to review these complaints upon opening the patient's next encounter.
- For each remaining complaint, the provider can choose:
- Keep: Adds the complaint to the current encounter.
- Deactivate: Removes the complaint from active status.
- Keep Tracks on for this patient: If the patient still needs separate Tracks, select this option to turn Tracks back on and preserve the separation.
You are only asked to manage active complaints on a patient's next visit after turning off tracks if that patient has additional active complaints that were not present on the most recent encounter. Otherwise, ChiroUp carries forward all active complaints into one unified set of notes.
At all following encounters:
- Patients will only have one “set” of notes, which will carry over from visit to visit every time, regardless of provider or treatment type. ChiroUp will populate new notes based on the information in the most recent note every time.

In this example, the patient’s most recent encounter was for the Right Upper Back and Shoulder complaint. After the clinic turned off Tracks, the patient was therefore asked about the Right Upper Back and Shoulder complaint at their next check-in.
The patient also has an active Lower Back complaint, but it was not part of their most recent encounter (it's from a different track than the one most recently used). As a result, the patient was not asked about the Lower Back complaint at check-in.
When the provider opens the encounter, ChiroUp prompts you to manage any active complaints that were not addressed during check-in. For the Lower Back complaint, the provider can:
- Deactivate the complaint if the patient no longer needs care for it.
- Note: Deactivating a complaint is a permanent action. Deactivated complaints cannot be reactivated.
- Keep the complaint active if the patient still needs care for it.
- Keep Tracks on for this patient if the patient needs their complaints managed on separate Tracks.
This ensures that turning off Tracks does not automatically remove a patient’s other active complaints. Instead, the provider can review and manage those complaints as appropriate for the patient’s current care.

‼️ Turning Tracks OFF for Individual Patients
Here's what happens when you turn tracks off for individual patients from the General tab of their patient record.
When you toggle off Use Tracks for an individual patient:
- Review the patient’s existing complaints. You’ll be prompted to manage the patient’s existing complaints so they can be combined into a single care workflow.
-
Choose which complaints to keep or deactivate. Any complaints you keep will be included in all future visits for this patient.
- Important: Deactivating a complaint is permanent. Once deactivated, a complaint cannot be reactivated.
- Existing Tracks become inactive. The patient’s previous Tracks are archived as Inactive and remain available for future reference.
Once you’ve managed the patient’s active complaints, Tracks will no longer be used for this patient. Moving forward, all kept complaints and SOAP information will carry forward from visit to visit, regardless of the provider or treatment type.
At all following encounters:
- Patients will only have one “set” of notes, which will carry over from visit to visit every time, regardless of provider or treatment type. ChiroUp will populate new notes based on the information in the most recent note every time.

❓What happens to all of my prior notes when I turn OFF Tracks?
Nothing! Your existing notes remain intact and unchanged. You can still edit, sign, and unsign those notes like normal. Turning OFF Tracks only changes how your notes are created and managed moving forward.
❓If I turn OFF Tracks, can I turn them back on?
Yes! You can enable Tracks again at any time after turning them OFF.
When Tracks are turned OFF (at the clinic level), ChiroUp inactivates your patients’ existing Tracks rather than deleting them. If you turn Tracks back ON before seeing the patient again, their existing Tracks will simply be reactivated.
If the patient is seen while Tracks are OFF or if you're managing Tracks on the individual level, turning Tracks back ON works differently. See Turning Tracks Back ON for details on what happens in these scenarios.
