Requesting and Generating Forms

Make collecting patient information easier with Custom Forms. Send forms directly to patients to complete, or generate them for your team to complete internally. Use your own forms, forms shared by other ChiroUp clinics, or build one from scratch.

This article walks you through how to request or generate a Custom Form. If you haven't built your form yet, check out our article on creating and editing Custom Forms first. 


Requesting vs Generating Forms

Custom Forms can be completed by patients, staff, or both. The way you use a form depends on who needs to complete it:

  • Patient-only: Send the form through Request Info for the patient to complete.
  • Patient + Staff: Generate the form from Patient Documents so staff can complete their fields before sending it to the patient.
  • Staff-only: Generate the form from Patient Documents for internal use.

Remember: If a form contains any User-completed fields, it must be generated from Patient Documents. Only forms completed entirely by the patient can be sent through Request Info.


Make a Form Available to Patients

Before you can send a form to a patient, enable the Send to patient setting.

1. In Custom Forms, open the form you want to make available.

2. Select the Settings tab.

3. Under Delivery & Storage, toggle Send to patient ON.


Requesting a Patient-Only Form

Patient-only forms can be requested directly through the Request Info field in ChiroUp. Within a patient's record, click on the Request Info icon. Then, select which form you'd like to Request and your delivery method.

You can also request forms from the Add Patient screen! 

 

 


Generating a Form with User-designated fields

Forms with any User-designated fields must be generated from Chart > Documents in a patient's record. 

1. Navigate to the Chart tab of a patient's record and then to Documents

2. Click Generate Form

 

3. Complete User-completed fields. 

  • If the form has Patient-completed fields, select your preferred send method to deliver the form to the patient.
  • If the form has no Patient-completed fields, choose Print or Save to Chart under Documents to complete the workflow internally.

Viewing Completed Forms 

Completed Forms are stored in Patient Documents

By default, ChiroUp saves completed Forms to the system Forms folder. You can choose a preferred Clinic-created folder in Custom Form Settings, and completed Forms will automatically be saved there instead.
