All communications are sent directly to a patient's guardian if they:

1) Are a minor (<18),

2) Have a guardianship documented in their patient record.

ChiroUp does not collect contact information for patients who are minors or patients who have legal guardians. All communications are always sent to a patient's guardian in the above-mentioned cases. 

To send digital surveys to guardians:

1) Navigate to Request info in the patient's record and select the desired survey(s). 

2) Select the guardian's preferred method of delivery and press "Send now."

*Please ensure guardian contact information is accurate for all applicable patients. You are responsible for ensuring all communications are sent to the correct mobile number and email.
