Exported Data Mapping

Upon request when ending your ChiroUp EHR subscription, ChiroUp will provide a data export of SOAP Notes and Chart uploaded files. This article explains how to navigate the exported files


Export Delivery 

When a clinic export is complete, a ZIP file containing all patient data will be uploaded to a secure Google Drive folder. The clinic is responsible for downloading this ZIP file. The file name will follow the format: “patient_charts_2025-01-01”

 

Folder Contents

Once unzipped, the folder will contain a list of all patients who were exported


Each patient’s folder is named using the ChiroUp Patient ID and Patient Name

Inside each patient folder will be one or both of the following subfolders:

  • encounters
  • Chart_entries

In addition, each patient folder will include a file named manifest.txt, which provides a detailed breakdown of all included records with the encounters and Chart_entries folders. Including: 

  • Patient Name, ID, and DOB
  • A categorized list of items under Encounters and/or Chart Entries (Files)
  • Each record's date, file path, and status (for example, signed or unsigned)

 

Locating a Specific Record

To find a specific note or entry, open the patient's folder and refer to the manifest to locate the desired record and its file path. 

Example File: encounters/2025-07-28_fee00419-21e8-4624-93b4-0039751a8c55/soap_note.txt

This means you should navigate to: 

  • The encounters folder
  • The 2025-07-28_fee00419-21e8-4624-93b4-0039751a8c55 subfolder
  • Open the soap_note.txt file

 

Notes: 

  • Encounter files are organized by the date and unique encounter ID
  • The same structure applies to manual chart entries
  • The manifest is the best reference for verifying file contents and identifying signed notes
